Visit us at NKHS.org Check out Life in the NEK of Vermont RESPONSIBILITIES & SCOPE Promote community integration of individuals and assist/encourage natural supports necessary.Counsel clients about the use of the National Suicide and Crisis Lifeline, 988, local hotlines, warm lines, mobile crisis, and other needed resources.Assess clients need/desire in accessing benefits, such as Medicaid and financial assistance.Develop partnerships with a variety of community services and supports.Contact any clients that have been discharged from an inpatient setting within 24 hours of discharge to connect them to services.Provide service coordination with a medical focus among internal and external providers.Reviews schedule daily and contacts clients that no show and reschedule immediately.
